Mutual Responsibility Contract

SBCC EOPS Spring 2025 - Student Responsibilities:

EOPS Students,
Once you have been officially admitted into the program, please review the information below. Then, scroll to the bottom of the page to complete the form and submit your electronic signature. This step is required before your EOPS services can be activated. Thank you!

  1. Complete the FAFSA each academic year.

  2. Meet with an EOPS Academic Counselor three times per semester to complete and/or update your SEP (Student Education Plan).
    Attendance at an EOPS orientation or workshop counts as one meeting.
    • First contact: Deadline: 2/28/25
    • Second contact: Deadline: 3/21/25
    • Third contact: Deadline: 4/18/25 (or attend an EOPS Orientation or Workshop by this deadline).

  3. Schedule and attend all counseling appointments on time.
    • Failure to meet with an EOPS counselor three times per semester may result in the termination of EOPS services, meal plans, book grants, vendor cards, and the EOPS grant.
    • Call or email the EOPS office at least 24 hours in advance if you need to cancel your academic appointment. Missing two scheduled academic counseling appointments per semester without proper notice may result in the termination of EOPS services.

  4. Notify your EOPS counselor of any changes to your educational goals so your SEP remains up to date.

  5. Meet with your Student Program Advisor (SPA) once per semester.
    • Deadline: 3/7/25.

  6. Maintain academic progress by enrolling in at least 12 units each semester (unless otherwise approved) and:
    • Achieve a semester and cumulative GPA of 2.0 or higher.
    • Complete at least 75% of the units attempted each semester.
    • DSPS students may enroll in fewer than 9 units with proper documentation from the DSPS office.

  7. Utilize tutoring or other student support services as recommended by EOPS counselors, SPAs, and staff.

  8. Update your personal information in MySBCC (e.g., name, address, phone number) and notify the EOPS Office promptly if changes occur.

  9. Respond to all communication from EOPS staff and peer mentors, including phone calls and emails.

  10. Refrain from inappropriate behavior, including verbal, emotional, or physical abuse toward EOPS staff.
    • Such behavior will result in removal from the EOPS program and/or SBCC.
    • Students must adhere to SBCC’s Student Code of Conduct.

  11. Comply with MRC (Mutual Responsibility Contract) requirements.

Failure to comply may result in suspension or termination of program services, including meal plans, book grants, vendor cards, EOPS grants, or other actions as determined by the EOPS Director.

By filling out and submitting the form below, I understand that the EOPS office will provide support services (student support services, academic counseling & advisement, tutoring, orientation, registration assistance for priority enrollment, mental health counseling, and assistance with university transfer) necessary to assist me in achieving my educational goals at SBCC. I understand that if I am accepted into the EOPS program, it is my responsibility to comply with the Mutual Responsibility Contract rules listed on this page.
